Once the installer is downloaded, you can use Disk Utility to format the USB thumb drive (as a single HFS+ partition), leaving the default 'Untitled' volume label is fine. If it does not appear in the App Store, check your purchased tab. Using the App Store, first acquire the version of OS X that you want to install (Mavericks, Yosemite, El Capitan, Sierra).
